What Diverticulitis Is and How It Develops

Diverticulitis is an inflammation or infection of small pouches called diverticula that can form in the colon. To understand this condition, it helps to know the structure of your digestive system. Your colon is a tube-like organ that removes water from waste and prepares it for elimination from your body. The walls of your colon are made of muscle and tissue layers.

Over time, small bulges can develop in weak spots along the colon wall. These bulges are the diverticula. For many people, having these pouches causes no problems at all—a condition called diverticulosis. However, when one or more of these pouches become inflamed or infected, the condition is called diverticulitis.

Research shows that diverticular disease affects a significant portion of the population. Studies indicate that by age 60, about 30% of people in Western countries have diverticulosis. By age 80, this number increases to nearly 50%. In younger people, diverticulitis is less common but can still occur.

The inflammation happens when stool or bacteria get trapped in a pouch. This can cause pain, usually on the lower left side of the abdomen (though location varies). Other symptoms may include fever, nausea, constipation, or diarrhea. Some people experience severe symptoms requiring hospitalization, while others have mild symptoms that improve with rest and diet changes.

Practical takeaway: Knowing that diverticula often cause no symptoms helps distinguish between simple diverticulosis (pouches without inflammation) and diverticulitis (inflamed pouches). This distinction matters for treatment decisions and dietary management.

Understanding Risk Factors and Prevention

Several factors contribute to the development of diverticulitis. While some risk factors cannot be changed, understanding them can help you make informed decisions about your health.

Age is one primary risk factor. Diverticulitis becomes increasingly common as people get older. The condition is rare before age 40 but becomes more frequent in people over 60. This happens partly because the colon wall weakens with age.

Genetics also plays a role. If family members have had diverticulitis, your risk may be higher. However, having a family history does not mean you will definitely develop the condition.

Lifestyle factors significantly influence risk. A diet low in fiber is strongly associated with diverticular disease. Fiber helps keep stools soft and moving through the colon regularly. Without enough fiber, stool can become hard and constipated, increasing pressure inside the colon and potentially contributing to pouch formation.

Physical inactivity increases risk as well. Regular movement helps your digestive system function properly. Studies show that people who exercise regularly have lower rates of diverticulitis compared to sedentary individuals.

Other factors that may increase risk include smoking, excessive alcohol consumption, obesity, and use of certain medications like nonsteroidal anti-inflammatory drugs (NSAIDs). Some research also suggests that diets high in red meat and processed foods may increase risk, while diets rich in vegetables and whole grains may lower it.

Practical takeaway: Since you cannot change your age or genetics, focus on modifiable factors. Increasing fiber intake, staying physically active, avoiding smoking, and limiting alcohol are actions within your control that research suggests may help reduce diverticulitis risk.

Fiber: The Foundation of Diverticulitis Prevention

Dietary fiber is consistently identified as the most important nutrient for preventing diverticulitis. Fiber is the part of plant foods that your body cannot fully digest. Instead of being broken down, fiber passes through your digestive system largely intact, playing crucial roles in digestive health.

There are two types of fiber: soluble and insoluble. Soluble fiber dissolves in water and forms a gel-like substance. This type is found in foods like oats, beans, apples, and barley. Insoluble fiber does not dissolve in water and adds bulk to stool. You'll find this in whole grains, vegetables, and wheat bran. Both types support digestive health, and ideally your diet should include both.

Fiber works to prevent diverticulitis in several ways. First, it keeps stool soft and bulky, making it easier to pass. This reduces the pressure needed for your colon to move stool along. Second, regular fiber consumption promotes regular bowel movements, preventing constipation. Third, fiber feeds beneficial bacteria in your colon, supporting a healthy microbiome.

Current dietary guidelines recommend 25 grams of fiber daily for adult women and 38 grams for adult men. However, most Americans consume only about 15 grams daily—less than half the recommended amount. Increasing fiber gradually is important because sudden increases can cause bloating and gas as your digestive system adjusts.

Good fiber sources include whole grain breads and cereals, legumes (beans, lentils, chickpeas), fruits with skin (apples, pears, berries), vegetables (broccoli, Brussels sprouts, carrots), nuts, and seeds. Gradually replacing white bread with whole wheat, choosing high-fiber cereals, and adding beans to meals are practical ways to increase intake.

Practical takeaway: Work toward meeting daily fiber recommendations by gradually adding more plant-based foods to your meals. Pair fiber increases with adequate water intake, as fiber works best when you're well-hydrated.

Foods to Include and Avoid for Prevention

Building a diet aimed at diverticulitis prevention involves both adding beneficial foods and limiting those that may increase risk. This approach is based on research examining dietary patterns and diverticular disease outcomes.

Foods to emphasize in your diet include vegetables of all types—both raw and cooked. Leafy greens, bell peppers, zucchini, squash, and root vegetables all provide fiber and nutrients. Whole fruits with skin intact offer both fiber and antioxidants. Berries, apples, pears, and oranges are particularly nutrient-dense choices. Legumes—including black beans, kidney beans, lentils, and chickpeas—are protein-rich and high in both soluble and insoluble fiber. Whole grains like brown rice, quinoa, whole wheat bread, and oatmeal provide sustained energy along with fiber. Nuts and seeds, including almonds, flaxseeds, and chia seeds, offer healthy fats alongside fiber.

Research suggests limiting certain foods may be beneficial, particularly red and processed meats. Studies show associations between high red meat consumption and increased diverticulitis risk. Processed meats like bacon, sausage, deli meats, and hot dogs fall into this category. These foods are often high in sodium and low in fiber. Similarly, refined carbohydrates—white bread, regular pasta, sugary cereals—lack the fiber of their whole grain counterparts.

High-fat foods, especially those combined with low fiber, may contribute to digestive issues. This includes fried foods, fatty cuts of meat, and creamy sauces. Sugary beverages and snacks offer calories without nutritional benefit and don't support healthy digestion. Alcohol, particularly in excess, has been associated with increased diverticulitis risk in some studies.

It's worth noting that individual tolerance varies. Some people with a history of diverticulitis find that certain foods trigger symptoms, while others tolerate them well. Keeping a food diary can help identify personal patterns.

Practical takeaway: Start by adding more vegetables, whole grains, legumes, and fruits to your current meals rather than focusing on elimination. Gradual dietary shifts are more sustainable than drastic changes.

Hydration and Other Lifestyle Factors

While fiber receives much attention in diverticulitis prevention discussions, water intake deserves equal emphasis. Fiber only works optimally when you're drinking adequate fluids. Water softens stool and helps it move through your colon more easily, reducing the straining that increases intra-colonic pressure.

General recommendations suggest drinking about 8 glasses (64 ounces) of water daily, though individual needs vary based on activity level, climate, and overall health. People who exercise regularly or live in hot climates may need more. A practical approach is to drink water consistently throughout the day rather than consuming large amounts at once. Listen to your body's thirst signals as well.
