What TB Testing Is and Why It Matters

Tuberculosis (TB) is a serious infection caused by bacteria that usually affects the lungs, though it can spread to other parts of the body. According to the World Health Organization, TB remains one of the top infectious diseases worldwide, with approximately 10 million new cases reported each year. In the United States, the CDC estimates there are around 2.3 million people with latent TB infection, meaning the bacteria are in their body but they don't feel sick yet.

TB spreads through the air when an infected person coughs, sneezes, or speaks. Not everyone exposed to TB bacteria gets sick. Some people develop active TB disease, where symptoms appear and the condition spreads. Others have latent TB infection, where the bacteria stay dormant and they show no symptoms. Understanding the difference between these two forms is crucial for knowing why testing matters.

TB testing helps identify who has TB infection before serious illness develops. When caught early, TB is treatable with medications taken over several months. Without treatment, active TB can cause severe lung damage, spread to other organs, and be fatal. Testing is particularly important for people who have been in close contact with someone who has TB, healthcare workers, people with weakened immune systems, and those living in congregate settings like shelters or correctional facilities.

The CDC recommends TB testing for specific groups based on their risk of exposure or developing disease. This risk-based approach means not everyone needs testing, but certain populations benefit significantly from knowing their TB status. A practical takeaway: if you've had close contact with someone diagnosed with TB, work in healthcare, or have symptoms like a persistent cough lasting more than three weeks, fever, night sweats, or weight loss, seeking TB testing information is an important step toward understanding your health.

Types of TB Tests Available

There are two main types of TB tests used in the United States: the tuberculin skin test (TST) and interferon-gamma release assays (IGRAs). Each test works differently and has different strengths depending on the individual being tested.

The tuberculin skin test, also called the Mantoux test, involves injecting a small amount of purified protein derivative (PPD) just under the skin on the forearm. A healthcare provider examines the injection site 48 to 72 hours later to measure any swelling. The amount of swelling indicates whether TB infection is present. This test has been used for over a century and remains widely available. However, it requires two visits—one for the injection and one for reading the results. People who have received a BCG vaccine (a TB vaccine used in many countries outside the US) may have a false positive reaction to the TST.

Interferon-gamma release assays are blood tests that measure how the immune system responds to TB antigens. The most common IGRA used in the US is the QuantiFERON-Gold In-Tube test. This blood test can be done in one visit, and results are available within 24 hours in many settings. IGRAs are not affected by BCG vaccination, making them more specific for TB infection detection. However, they may be less sensitive in young children under five years old and in people with severe immune system weakening.

Some facilities may use both tests together for certain situations. For example, if someone has conflicting results or uncertain exposure history, both tests might provide clearer information. The choice between tests often depends on what's available at the testing location, the person's age, their vaccination history, and their immune status. A practical takeaway: when you contact a testing facility, ask which type of test they offer and understand that different tests may be recommended based on your specific circumstances, age, and health history.

Who Should Consider Getting Tested for TB

TB testing recommendations from the CDC focus on people with higher risk of TB infection or disease. Understanding whether you fall into a group that benefits from testing helps you make informed decisions about your health. The CDC prioritizes testing for several specific populations.

People with close contact to someone with active TB disease should be tested. Close contact typically means spending significant time in the same room with an infected person during their contagious period. Healthcare workers and others in occupational settings with TB exposure benefit from regular testing, often annually or based on workplace policies. Incarcerated individuals and those in custody have higher TB rates and are routinely tested. People experiencing homelessness or living in shelters, where crowded conditions increase transmission risk, are recommended for testing. Immigrants, refugees, and people from countries with high TB rates may have higher infection rates and are often screened.

People with medical conditions that weaken the immune system should discuss TB testing with their healthcare provider. These conditions include HIV infection, diabetes, chronic kidney disease, malnutrition, and conditions requiring immunosuppressive medications. People with symptoms suggestive of TB—such as a cough lasting more than three weeks, chest pain, hemoptysis (coughing up blood), fever, chills, or unexplained weight loss—should be tested promptly.

Young children under five who have been exposed to someone with active TB are tested, as they have higher risk of developing severe TB disease. People who inject drugs may have increased TB exposure risk in certain settings. Additionally, healthcare systems sometimes conduct screening in high-risk facilities like nursing homes, dialysis centers, and homeless shelters to identify cases early.

A practical takeaway: review this list with your own circumstances in mind. If you recognize yourself in any of these categories, learning more about TB testing options in your area is a reasonable next step. You don't need to fit into these groups to obtain testing—many people request tests for personal peace of mind or based on specific exposure incidents. Testing is one tool for understanding your health status.

What Happens During and After TB Testing

Understanding the testing process removes uncertainty and helps you know what to expect. The experience differs slightly depending on which test you receive, but both are quick, minimally invasive procedures.

For the tuberculin skin test, you'll visit a healthcare facility where a nurse or medical assistant injects PPD just under the skin on your inner forearm. The injection itself feels like a minor pinch. You receive instructions to keep the area clean and dry and return in 48 to 72 hours for the reading. The healthcare provider measures the size of any hardened swelling (induration) at the injection site using a ruler. The measurement in millimeters, combined with your risk factors, determines whether the result is positive, negative, or uncertain. During the wait period between injection and reading, you can resume normal activities. Avoid scratching, rubbing, or covering the injection site.

For the interferon-gamma release assay, you have a standard blood draw where a technician collects a small amount of blood, typically from your arm. The blood is placed in special tubes that contain TB antigens. The sample is sent to a laboratory where it's incubated and tested for interferon-gamma, a substance your immune cells produce if TB infection is present. Results typically come back within 24 to 48 hours, though this varies by laboratory and facility. You generally receive results by phone call or at a follow-up appointment.

After receiving test results, understanding what they mean is important. A negative result generally indicates no TB infection is present, though timing of exposure and immune status can affect accuracy. A positive result means TB infection is present but doesn't necessarily mean you have active TB disease. Many people with positive test results have latent TB and never develop symptoms. Your healthcare provider will discuss what your results mean for your specific situation and whether further testing or evaluation is needed.

A practical takeaway: on the day of a skin test, wear loose, short-sleeved clothing to make the injection and reading easier. For either test, bring your insurance card or information about your ability to pay, and ask about how you'll receive results and what happens next based on your test outcome.

Understanding Test Results and Next Steps

TB test results require interpretation based on your individual risk factors, symptoms, and exposure history. A positive TB test doesn't automatically mean you have active TB disease, and understanding the difference is critical for appropriate next steps.

A negative TB test result typically means you don't have TB infection. However, timing matters. If you were very recently exposed to TB (within the past three to eight weeks), the test may not yet show infection. In these cases, healthcare providers sometimes recommend retesting after an interval to allow sufficient time for the immune response to develop. People with severely weakened immune systems may also have false negative results because their immune response is too weak to be detected.

A positive TB test result indicates TB infection is present. According to CDC data,
