Overview of Substance Abuse Treatment in Texas

Texas has a wide range of treatment options for people struggling with substance abuse. According to the Substance Abuse and Mental Health Services Administration (SAMHSA), in 2021, about 1.3 million people in Texas needed treatment for substance use disorders, but only a fraction received care. This guide describes the types of treatment programs available across the state, how they work, and what to expect when exploring options.

Treatment for substance abuse is not one-size-fits-all. Different people need different approaches depending on their situation, the substances they use, their medical history, and their personal circumstances. Some people benefit from inpatient residential programs where they stay at a facility for several weeks or months. Others do well with outpatient programs where they attend treatment sessions while living at home. Many people use a combination of treatments.

The state of Texas oversees substance abuse treatment through several agencies, including the Texas Health and Human Services Commission (HHSC) and the Texas Department of State Health Services. These agencies maintain networks of treatment providers and programs throughout the state. Major cities like Houston, Dallas, Austin, and San Antonio have many treatment options, while rural areas may have fewer facilities but still offer programs through regional health centers.

Understanding the landscape of treatment in Texas is the first step toward exploring what might work for your situation. This guide breaks down the main categories of treatment, explains how different programs function, describes what happens during treatment, and points toward resources where you can find providers. The information here is educational and meant to help you understand your options, not to make decisions for you.

Practical Takeaway: Treatment works differently for different people. Learning about the various types of programs available in Texas helps you understand what options exist and what questions to ask when contacting providers.

Types of Treatment Programs Available

Texas offers several distinct categories of substance abuse treatment, each designed to meet different needs. Understanding these categories helps you think about what structure and level of support might fit a particular situation.

Inpatient/Residential Programs: These programs require people to live at a treatment facility, usually for 28 days to several months. Residents receive structured treatment throughout the day, including individual counseling, group therapy, medical care, and educational sessions about addiction. Residential programs are often recommended for people with severe addictions, those who have failed outpatient treatment before, people with co-occurring mental health conditions, or anyone in crisis. Examples include state-run residential treatment centers and private facilities. According to SAMHSA data, about 23 percent of people in Texas who received substance abuse treatment in recent years were in residential programs.

Outpatient Programs: Outpatient treatment allows people to live at home while attending therapy and counseling sessions at a treatment center. Standard outpatient programs typically involve attending sessions a few times per week for a few hours at a time. Intensive outpatient programs (IOPs) are more frequent—usually three to five days per week for three to four hours per day—and provide a middle ground between residential and standard outpatient care. Outpatient programs work well for people with mild to moderate addictions, those with family or work responsibilities, people who prefer to stay in their community, or those transitioning out of residential treatment.

Medication-Assisted Treatment (MAT): This approach combines medications with counseling and behavioral therapies. For opioid addiction, medications like methadone, buprenorphine, and naltrexone reduce cravings and withdrawal symptoms. For alcohol addiction, medications such as naltrexone, acamprosate, and disulfiram may be used. MAT is often delivered through outpatient clinics and has strong research support. Texas has numerous methadone clinics and buprenorphine prescribers, though availability varies by region.

Partial Hospitalization Programs (PHP): These programs provide intensive treatment during the day—often 20 or more hours per week—but allow people to return home at night. They bridge the gap between inpatient and standard outpatient care and work well for people who need significant support but have stable living situations.

Peer Support and Recovery Programs: While not clinical treatment, programs like 12-step groups (Alcoholics Anonymous, Narcotics Anonymous), SMART Recovery, and other peer-led initiatives provide ongoing support. Many people combine these with clinical treatment.

Practical Takeaway: Consider your living situation, work or school commitments, addiction severity, and support system when thinking about which type of program structure might work. More intensive residential programs aren't necessarily "better"—the right fit depends on your individual circumstances.

SAMHSA National Helpline: The Substance Abuse and Mental Health Services Administration runs a free, confidential helpline at 1-800-662-4357, available 24/7. This service is not run by the state but by a federal agency, and operators can provide information about treatment providers in Texas. They can discuss what type of program might fit a situation and connect you with local resources. This helpline does not require personal information and is not affiliated with law enforcement.

Texas HHSC Substance Abuse Treatment Locator: The Texas Health and Human Services Commission maintains a database of treatment providers. You can visit their website to search for programs by location, type of treatment, and specialty services. The HHSC oversees both state-funded and contracted treatment programs.

SAMHSA Treatment Locator Online: At findtreatment.gov, you can search for treatment facilities nationwide, including Texas. The tool allows you to filter by location, type of treatment, and whether programs accept insurance or offer sliding-scale fees.

Local Health Departments and Community Health Centers: County and city health departments often have information about treatment options in their area. Federally Qualified Health Centers (FQHCs) throughout Texas provide or can refer to substance abuse treatment. A search for "community health center" plus your city name can help you locate these facilities.

Insurance Provider Networks: If you have health insurance, your insurance company maintains a list of in-network treatment providers. Calling the number on your insurance card is a direct way to find programs that your plan covers.

Hospital Emergency Departments: If someone is in crisis, hospital ERs can provide crisis stabilization and referrals to treatment. They can also provide information about programs in the area.

Texas Addiction Recovery Resources (TARR): This advocacy organization provides information and resources about treatment options across Texas.

Questions to Ask When Contacting Programs:

• What type of treatment do you offer (inpatient, outpatient, PHP, MAT)?
• What substances does your program treat?
• How long is the typical program duration?
• Do you treat co-occurring mental health conditions?
• What is your current wait time to start?
• What insurance do you accept, or do you offer sliding-scale fees?
• What is your aftercare or continuing care plan?
• Do you offer any specialized services (adolescent programs, women's programs, trauma-informed care)?
• What staff credentials do your counselors have?

Practical Takeaway: You have multiple pathways to find treatment. Start with a resource like the SAMHSA helpline or online treatment locator, then contact programs directly with specific questions about their services and how to move forward.
