Treatment in South Carolina generally falls into several categories: inpatient or residential programs, outpatient programs, medication-assisted treatment, and support groups. Inpatient programs require people to stay at a facility for a set period, usually ranging from 28 days to several months. Outpatient programs allow people to live at home while attending treatment sessions at scheduled times. Medication-assisted treatment combines medications with counseling and behavioral therapies. Support groups bring people together to share experiences and encourage each other.

According to the South Carolina Department of Alcohol and Other Drug Abuse Services (DAODAS), there are more than 300 licensed substance abuse treatment facilities and programs throughout the state. These programs serve thousands of people each year. The types of treatment available reflect different philosophies and approaches to recovery, meaning people with different needs and circumstances may find options that work for their situation.

Many facilities in South Carolina are accredited by national organizations that set quality standards. Accreditation means the program has met certain requirements for staff training, safety, and treatment methods. Not all programs are accredited, but accreditation can be one factor to consider when learning about options.

Treatment programs in South Carolina also differ in cost structure. Some receive state funding, some operate through insurance, and some offer sliding scale fees based on income. Understanding these differences helps when exploring what might be available in a particular area or circumstance.

Practical Takeaway: South Carolina's treatment landscape includes many different program types and settings. Learning the basic differences between inpatient, outpatient, and medication-assisted approaches provides a foundation for understanding what options exist.

Inpatient and Residential Treatment Programs

Inpatient or residential treatment means staying at a facility while receiving care. These programs provide a structured environment where people live, receive medical supervision, attend therapy sessions, and participate in group activities. The length of stay typically ranges from 28 days to 90 days, though some programs run longer.

Residential programs are designed for people with more severe addictions, those with co-occurring mental health conditions, or those who have tried outpatient treatment without success. Living at the facility removes people from their usual environment, which can reduce access to substances and help people focus on recovery. Staff are available 24 hours a day, which means medical emergencies or crisis situations can be addressed immediately.

Most inpatient programs in South Carolina follow similar daily structures. A typical day might include morning meetings, individual therapy sessions with a counselor, group therapy, educational classes about addiction and recovery, meals, and recreational activities. Evenings often include peer support group meetings. Some programs also offer family therapy sessions so loved ones can learn about addiction and how to support recovery.

South Carolina's inpatient facilities vary in setting and amenities. Some are located in hospitals and provide medical detoxification services. Others operate as standalone residential centers. Facilities may range from basic accommodations to more comfortable settings. The quality and cost of facilities can differ significantly across the state.

Medical detoxification is often the first phase of inpatient treatment. When someone stops using certain substances, their body may experience withdrawal symptoms that are uncomfortable or even dangerous. Medical detoxification provides medication and monitoring to manage these symptoms safely. Not all inpatient programs offer medical detoxification on-site, so this is an important question to ask when learning about specific facilities.

Insurance coverage for inpatient treatment varies. Many health insurance plans cover some or all of inpatient treatment costs. Medicaid in South Carolina also covers inpatient treatment for eligible individuals. Some programs offer self-pay options with sliding scale fees based on income.

Practical Takeaway: Inpatient programs provide round-the-clock care and are useful for people with severe addiction or multiple barriers to recovery. When learning about these programs, key questions include program length, whether medical detoxification is offered, what therapy approaches are used, and cost structure.

Outpatient Treatment and Counseling Services

Outpatient treatment allows people to receive care while living at home and maintaining work, school, or family responsibilities. People attend treatment sessions at a facility or clinic on a regular schedule—perhaps several times per week or a few times per month, depending on the program's intensity level. Outpatient programs range from standard outpatient services to intensive outpatient programs (IOP) and partial hospitalization programs (PHP).

Standard outpatient programs typically involve attending one or more counseling sessions per week. These sessions focus on understanding triggers for substance use, developing coping skills, and working toward recovery goals. Standard outpatient works well for people with mild to moderate substance use concerns or for those transitioning from more intensive treatment.

Intensive Outpatient Programs (IOP) require more time commitment, typically 9 to 20 hours per week. People attend sessions several days per week for several hours each day. IOP provides more structure and support than standard outpatient while still allowing people to live at home. IOP is suitable for people with moderate to serious addiction issues, those managing co-occurring mental health conditions, or those transitioning from inpatient treatment.

Partial Hospitalization Programs (PHP) are even more intensive, often running 20 to 40 hours per week during daytime or evening hours. PHP provides a level of care between inpatient and IOP. People typically attend five days per week. This option works for people who need significant support but do not require 24-hour supervision or medical monitoring.

South Carolina's outpatient programs typically include individual counseling with a licensed therapist or counselor, group therapy with peers, educational sessions about addiction and recovery, and sometimes family therapy. Many programs also refer people to or host peer support group meetings. Some offer medication-assisted treatment alongside counseling.

Outpatient services are available throughout South Carolina, including in rural and urban areas, though availability varies by location. Community mental health centers, substance abuse treatment clinics, and private practices all offer outpatient services. Many outpatient programs accept Medicaid, private insurance, or offer sliding scale fees for uninsured individuals.

Practical Takeaway: Outpatient programs work well for people with less severe addiction, stable housing and support systems, or those who prefer to remain in their community. Understanding the difference between standard, intensive, and partial hospitalization outpatient options helps identify which level of care might fit a particular situation.

Medication-Assisted Treatment Options

Medication-assisted treatment (MAT) combines medications that reduce cravings or withdrawal symptoms with counseling and behavioral therapy. This approach is particularly effective for opioid and alcohol use disorders. In South Carolina, several medications are available through MAT programs.

For opioid addiction, three main medications are used: methadone, buprenorphine, and naltrexone. Methadone is a synthetic opioid that prevents withdrawal symptoms and reduces cravings. People typically visit a methadone clinic daily or several times per week to receive their dose. Buprenorphine is a partial opioid agonist that also prevents withdrawal and reduces cravings, but with lower overdose risk than methadone. Buprenorphine can be prescribed in office-based settings, making it more accessible in some areas. Naltrexone blocks opioid effects and works differently than methadone or buprenorphine; it requires the person to be fully detoxified first.

For alcohol use disorder, medications include naltrexone, acamprosate, and disulfiram. Naltrexone reduces cravings for alcohol. Acamprosate helps maintain abstinence by reducing post-acute withdrawal symptoms. Disulfiram causes unpleasant reactions if alcohol is consumed, providing a deterrent. Each medication works through different mechanisms and may work better for different people.

South Carolina has methadone clinics in major cities and some smaller areas. The availability of office-based buprenorphine has increased in recent years, with prescribers in hospitals, community health centers, and private practices. Not all areas have equal access to these services, particularly in rural regions.

MAT programs combine medication with counseling and support. The counseling component is essential—medication alone is not considered complete treatment. Counselors
