Understanding Hammertoes: What They Are and Why They Develop

A hammertoe is a deformity where one or more of your toe joints bend abnormally, usually at the middle joint. The affected toe takes on a hammer-like shape—hence the name. According to the American Academy of Orthopaedic Surgeons, hammertoes affect millions of people in the United States, with women experiencing them more frequently than men, partly due to footwear choices.

Hammertoes develop through several mechanisms. The most common cause is an imbalance in the muscles and tendons that control toe movement. When these soft tissues become weak or tight, they pull the toe joint out of alignment. This often happens gradually over months or years. Certain factors increase your risk of developing hammertoes:

• Wearing tight, narrow shoes that compress the toes
• High heels that push toes forward into cramped spaces
• Arthritis that affects joint flexibility
• Toe injuries that heal incorrectly
• Family history of hammertoes or other foot deformities
• Neuromuscular conditions affecting muscle control
• Flat feet or high arches that change how weight distributes across toes

Early-stage hammertoes are often flexible, meaning the joint can still straighten manually. Advanced hammertoes become rigid, with the joint permanently bent. Catching a hammertoe early, before it becomes rigid, significantly improves your options for correction at home. Pain typically develops where the bent joint rubs against shoe material, creating pressure points and calluses on top of the toe or at the tip.

Practical takeaway: Examine your toes regularly. If you notice one bending abnormally but it still straightens when you gently push it, you have a flexible hammertoe that responds better to home treatment methods. Document when you first noticed the bending and what activities make it worse.

Footwear Changes: The Foundation of Home Treatment

The single most important step in treating hammertoes at home involves changing how you dress your feet. Your shoes either support toe alignment or work against it. Research from podiatry clinics shows that switching to appropriate footwear produces visible improvements in flexible hammertoes within 4 to 8 weeks for many people.

Look for shoes with these specific characteristics:

• Wide toe box: The front of the shoe should provide at least half an inch of space beyond your longest toe. Your toes should not touch the shoe's end when you stand. A toe box width measurement of at least 3.5 to 4 inches works for most people.
• Adequate height: The shoe should have enough vertical space so the top doesn't press down on bent toes. Many people need shoes that are at least 1.5 inches taller in the toe area than standard styles.
• Soft upper material: Leather, mesh, or canvas that gives slightly when pressure is applied protects tender areas better than rigid synthetic materials.
• Minimal heel: Heels higher than 1 to 1.5 inches shift weight forward onto your toes, worsening hammertoe deformity. Flat or low-heel options reduce this pressure significantly.
• Flexible sole: The shoe should bend where your foot naturally bends, not resist foot movement.

Avoid shoes marketed as "narrow," "sleek," or "tapered." These descriptions indicate a constrictive toe box. Similarly, avoid styles where the toe box narrows significantly toward the tip—this design concentrates pressure on your toes. Many athletic shoes, walking shoes, and casual brands offer wide-toe-box options that function well for hammertoe management.

Beyond regular shoes, consider therapeutic options. Sandals with contoured footbeds and open tops eliminate toe compression entirely. Some people wear sandals during home hours and switch to supportive shoes only when going outside. Custom shoe inserts (orthotics) can be adjusted to support toe position, though basic inserts cost $50 to $150 while custom versions run higher.

Practical takeaway: Measure your current shoes' toe box width and height. Try on at least five different brands known for wide toe boxes—Hoka, New Balance, Clarks, and Vionic make models specifically designed for foot issues. Wear the new shoes for at least two hours daily to allow your foot to adjust, then gradually increase wear time.

Stretching and Strengthening Exercises for Toe Muscles

Flexible hammertoes respond to targeted exercises that lengthen tight muscles and strengthen weak ones. Physical therapists recommend performing these exercises daily for 5 to 10 minutes. Studies on toe-focused physical therapy show that consistent exercise can reduce hammertoe severity and prevent progression in early stages.

Towel Scrunches: Spread a hand towel flat on a smooth floor. Place your barefoot on the towel with your heel on the ground. Curl your toes downward to bunch the towel toward you, then relax. Repeat 10 to 15 times per foot. This exercise strengthens the small muscles that support toe alignment and works the tendons on the bottom of your foot.

Toe Stretches: While seated, use your fingers to gently grasp the bent toe and slowly pull it straight. Hold this position for 15 to 30 seconds without forcing—you should feel a mild stretch, not pain. Repeat three times per toe. Do this twice daily. Never force a toe into a position that causes sharp pain, as this can cause tissue damage.

Marble Pickup: Place small marbles or pebbles on the floor. Using only your toes, try to pick up each marble and drop it into a cup. This challenges toe coordination and strengthens muscles. Start with 10 marbles and work up to 20 or more. This exercise is particularly effective because it mimics natural toe gripping motions.

Toe Spread Exercise: Sit with feet flat on the floor. Consciously spread your toes apart as far as possible, holding each spread for 5 seconds. Repeat 10 times. This teaches toe muscles to work independently and counteracts the bunching position of hammertoes.

Resistance Band Work: Loop a resistance band around the ball of your foot. While seated with your leg extended, use your toes to push the band away (plantarflexion) and then pull it toward you (dorsiflexion). Perform 10 repetitions in each direction, twice daily. A light resistance band works best for beginners—you should feel mild effort, not strain.

Practical takeaway: Start with towel scrunches and toe stretches, performing each once daily for one week. Add one new exercise each week so your foot gradually adapts. Keep a simple checklist marking off each completed exercise session. Many people find that morning and evening routines work best for consistency.

Taping, Splinting, and Support Devices

Temporary support devices hold your toe in a straighter position while you sleep and throughout the day, reducing joint stress and supporting muscle recovery. These methods work best on flexible hammertoes and can produce noticeable changes within 2 to 3 weeks of consistent use.

Taping Methods: Athletic tape or medical tape can gently pull a hammertoe into a straighter position. The process involves anchoring the tape to the side of your foot, then running it under your bent toe and back to the other side, creating a slight upward pull. This should feel supportive but not tight enough to cut off circulation. Replace tape daily, as it loses adhesiveness and effectiveness. Some people tape their toes before wearing shoes and remove it at night, while others tape continuously, changing it once daily.

Toe Splints and Straighteners: Various devices specifically designed for hammertoe management are available online and through medical supply stores. These include fabric sleeves that slip over individual toes with internal supports, plastic shell splints that hold toes straight, and gel toe spacers that both separate and support t
