Understanding CT Scans With Contrast: What You Should Know

A CT scan, also called a computed tomography scan or CAT scan, takes detailed pictures of the inside of your body. The scanner rotates around you and takes many X-ray images from different angles. A computer combines these images to create cross-section pictures of your organs, bones, and tissues.

When contrast is added to a CT scan, it makes certain areas of your body show up more clearly on the images. Contrast is a special dye or liquid that highlights blood vessels, organs, and other structures. This helps doctors see problems more easily, such as tumors, infections, bleeding, or blockages. According to the American College of Radiology, about 35 percent of all CT scans use some form of contrast material.

There are two main types of contrast used in CT scans:

• Oral contrast: You drink this liquid before the scan. It typically tastes chalky and may be flavored. Oral contrast stays in your stomach and bowels to help visualize these organs.
• Intravenous (IV) contrast: A technician injects this through a vein in your arm using a needle. It travels through your bloodstream and highlights blood vessels and organs. This type works faster and is used for many types of scans.

Your doctor decides which type of contrast you need based on what part of your body they want to examine. Some scans use both types together. The contrast passes through your body naturally over the next 24 hours and leaves your system through your urine or bowel movements.

Practical Takeaway: Understanding the purpose of contrast helps you prepare mentally for your scan. Contrast makes images clearer, which helps your doctor give you a more accurate diagnosis. Knowing which type you'll receive helps you know what to expect during your appointment.

Pre-Scan Preparation: What Your Doctor Will Tell You

Before your CT scan with contrast, your medical team will give you specific instructions to follow. These instructions are important because they affect the quality of your images and your safety during the procedure. Most facilities will call you 24 hours before your appointment or send you written instructions by mail or email.

For scans using oral contrast, you will typically need to fast (not eat or drink) for a certain period before your appointment. The fasting window is usually 4 to 6 hours before your scan time, though this can vary. Your doctor's office will tell you exactly when to stop eating and drinking. You may be told to drink the oral contrast liquid starting 1 to 2 hours before your scan appointment. You'll drink it in several portions, finishing the last dose about 15 to 20 minutes before your scan begins.

For scans using IV contrast, fasting requirements may be different. Some facilities ask you to avoid food for 4 hours beforehand, while others have no fasting requirement. Always follow the instructions from your specific medical facility, as practices vary.

If you take regular medications, ask your doctor whether you should take them on the day of your scan. Most medications are fine to take, but some need to be timed carefully around your scan. If you take diabetes medication, you may need special guidance about when to take your next dose after the scan. If you take metformin (a common diabetes medication), your doctor may ask you to stop taking it for 48 hours after the scan if you receive IV contrast, because the combination can affect your kidneys. This is especially important if you have kidney problems.

You should also tell your medical team about any allergies you have, especially if you're allergic to shellfish or iodine. Although rare, some people have reactions to contrast material. Knowing your allergies helps your doctor choose the safest type of contrast for you or take preventive steps if needed.

Practical Takeaway: Write down all pre-scan instructions when your doctor's office provides them, and call immediately if you don't understand something or have questions about your medications. Following these instructions carefully leads to better scan results and fewer complications.

Understanding Contrast Reactions: What Is Rare but Worth Knowing

Allergic reactions to CT contrast happen infrequently, but you should know the facts. According to medical literature, severe allergic reactions to iodine-based contrast occur in approximately 1 in 1,000 to 1 in 100,000 people. Most reactions are mild and easily treated. Very serious reactions are even rarer.

Mild reactions may include itching, a slight rash, or a warm feeling spreading through your body. These usually go away on their own within a few minutes. Moderate reactions might include hives, swelling of the lips or throat, or mild vomiting. Severe reactions are very uncommon but could include difficulty breathing, severe swelling, or a drop in blood pressure.

The medical staff monitoring you during your scan are trained to recognize and treat reactions immediately. They will ask about your medical history and any previous reactions to contrast before your scan. If you have had a reaction to contrast in the past, tell your doctor well before your appointment. You may be given premedication (usually steroid pills and antihistamine) about 12 hours and 1 hour before your scan to reduce the risk of another reaction.

People with certain health conditions should mention them to their doctor before the scan. If you have kidney disease, diabetes, multiple myeloma, or heart disease, let your medical team know. These conditions can affect how your body handles contrast material. Your doctor may check your kidney function with a blood test before your scan if you have risk factors. This blood test measures creatinine levels, which show how well your kidneys are working.

Dehydration can increase the risk of problems with contrast, so staying well-hydrated before and after your scan is important. Drink water regularly on the day of your scan unless your doctor told you not to.

Practical Takeaway: Reactions to contrast are uncommon, and medical staff are prepared to handle them. By being honest about your medical history and following pre-scan instructions, you help your medical team keep you safe and get the images they need.

The Day of Your Scan: What to Expect From Start to Finish

On the day of your CT scan, plan to arrive 15 minutes early. This gives you time to check in, ask final questions, and get settled. Bring your insurance card, photo ID, and any paperwork your doctor's office sent you. Wear comfortable, loose-fitting clothing. You may be asked to change into a hospital gown, so avoid clothing with metal buttons, zippers, or snaps if possible.

When you arrive, a technician will take you to a preparation area. If you're receiving IV contrast, a nurse will place an IV line in your arm before you enter the scanning room. The nurse will explain what will happen and may give you a call button to use if you need to communicate during the scan. If you're receiving oral contrast, you'll have already finished drinking it at home or in the waiting room.

In the scanning room, you'll lie down on a flat table that slides into a large, tube-shaped machine. The CT scanner looks like a big donut, and the table moves through the center opening. The scan itself is painless. You'll hear some buzzing or clicking sounds as the scanner works. The actual scanning usually takes only 10 to 30 seconds, though the entire visit typically lasts 30 to 60 minutes.

If you receive IV contrast, the technician may tell you that you'll feel a warm sensation or metallic taste in your mouth as the contrast enters your bloodstream. This is normal and passes quickly. Some people feel like they're going to urinate, but this sensation also passes. These feelings don't mean something is wrong.

During the scan, you'll need to hold very still and follow breathing instructions. The technician will tell you when to breathe in, hold your breath, or breathe normally. Holding still helps prevent blurry images. If you get anxious in enclosed spaces, mention this to your technician before the scan. Many facilities will keep the lights on or let you listen to music to help you feel more comfortable.

After the scan ends, the table slides out of the machine. You can get up and go about your day. If you had an IV, the nurse will remove it before you leave. You'll typically receive results within 24
