Understanding Mobility Support Programs

Mobility programs are services and resources designed to help people with movement limitations, disabilities, or age-related physical changes get around safely and independently. These programs exist at federal, state, and local levels, each serving different purposes and populations. Understanding what programs exist is the first step in learning what resources might be available to you or someone you care for.

According to the 2019 American Community Survey, approximately 27 million Americans reported having some form of mobility difficulty. This includes people with temporary injuries, permanent disabilities, chronic health conditions, and older adults experiencing age-related changes. The range of available programs reflects this diversity of need.

Mobility support falls into several broad categories: financial programs that help pay for mobility devices or modifications, transportation services that provide rides or vehicle access, medical equipment programs that loan or provide devices, and informational resources that teach people about their options. Some programs focus specifically on people with disabilities, while others serve seniors, low-income individuals, or people recovering from injury.

Programs operate through different systems. Medicare and Medicaid provide coverage for certain mobility devices under medical circumstances. Veterans may access benefits through the Department of Veterans Affairs. State vocational rehabilitation agencies serve working-age people with disabilities. Community organizations, nonprofits, and local governments often run transportation services and equipment loan programs. Transportation services might include paratransit (specialized bus services for people who cannot use regular public transportation), volunteer driver programs, or subsidized taxi services.

Practical takeaway: Start by identifying which category of need matches your situation—whether you need a device, transportation, payment support, or information—since different programs serve different needs.

Transportation Programs and Services

Transportation is one of the most common mobility challenges. People with mobility limitations often struggle to reach medical appointments, grocery stores, and community activities. Multiple transportation programs exist to address this gap, though availability varies by location.

Paratransit services operate in most metropolitan areas across the United States. These are specialized bus systems required by the Americans with Disabilities Act (ADA) to serve people who cannot use fixed-route public transportation due to disability. Paratransit typically requires advance reservations, operates along similar routes and times as regular buses, and charges fares comparable to standard transit. The service is designed for people whose disabilities prevent them from using conventional buses, even with assistance. Each city manages its own paratransit system, so services and costs differ by location. For example, a person with severe arthritis who cannot board a standard bus, or someone using a wheelchair, might use paratransit services.

Volunteer driver programs operate in thousands of communities nationwide. These services connect older adults and people with disabilities to volunteer drivers who provide free or low-cost rides to medical appointments, shopping, and community events. Organizations like Senior Corps programs coordinate many of these efforts. According to the National Volunteer Transportation Center, volunteer driver programs collectively provide over 120 million rides annually in the United States.

Medical transportation programs help people reach healthcare appointments. Some Medicaid programs provide non-emergency medical transportation (NEMT) services. Veterans can access transportation through VA medical centers for appointment-related trips. Many hospitals and health systems operate their own transportation services for patients. Some community health centers partner with local transportation providers to help patients reach care.

Subsidized taxi and ride-sharing programs exist in certain areas. Some cities use public funding to provide discounted taxi vouchers for seniors and people with disabilities. A few Medicaid programs have begun experimenting with ride-sharing services like Uber or Lyft for medical transportation, though this remains limited.

Practical takeaway: Contact your local public transportation agency or Area Agency on Aging to learn what specific transportation services operate in your community, since options vary significantly by location.

Mobility Devices and Equipment Programs

Mobility devices—wheelchairs, walkers, canes, scooters, crutches, and other equipment—help millions of people move independently. These devices can be expensive, ranging from $50 for a basic cane to $25,000 or more for a power wheelchair. Programs exist to help reduce this cost burden.

Medicare covers mobility devices when medically necessary and prescribed by a physician. Specifically, Medicare Part B covers items classified as "durable medical equipment" (DME). This includes standard wheelchairs, power wheelchairs, walkers, canes, crutches, and certain scooters. Coverage depends on a physician documenting that the device is medically necessary. The beneficiary typically pays 20% of the approved cost after meeting the Part B deductible, while Medicare covers 80%. A standard manual wheelchair might cost $1,000 to $3,000, with Medicare covering most of that expense for a covered beneficiary.

Medicaid programs in all states provide coverage for mobility devices, though the specific items covered and approval processes vary by state. Some states have very comprehensive coverage, while others cover only basic equipment. A person using Medicaid in one state might have different coverage options than someone in another state.

The Veterans Health Administration provides mobility devices to eligible veterans. The VA may provide wheelchairs, prosthetics, orthopedic devices, and other specialized equipment based on service-connected disabilities or medical need.

Equipment loan programs operate through nonprofits, hospitals, and community organizations. These programs loan mobility devices temporarily, which can be valuable when recovering from surgery or injury. The National Association of Medical Equipment Suppliers maintains a directory of providers. State vocational rehabilitation agencies sometimes provide equipment for working-age people with disabilities who need it for employment.

Donation and refurbishment programs obtain used equipment and make it available to people who cannot purchase new devices. Organizations like the National Council on Independent Living, various disability organizations, and community groups coordinate these efforts. Some hospitals have programs specifically for patients leaving care who need equipment.

Practical takeaway: If you have Medicare or Medicaid, speak with your healthcare provider about whether a mobility device is medically necessary—this documentation is required for coverage, and discussing options with your doctor is an important first step.

Home Modifications and Accessibility Resources

Physical changes to a home—ramps, grab bars, widened doorways, accessible bathrooms, stair lifts, and other modifications—enable people with mobility limitations to navigate their living spaces safely. Home modifications can cost from a few hundred dollars for basic items to $20,000 or more for major renovations. Several programs help offset these costs.

The Community Development Block Grant program provides federal funding to states and local governments for various community development purposes, including home modifications for low-income people and people with disabilities. Each community administers this differently, so a person in one county might find programs while a similar person in another county has fewer options.

HUD's Section 504 program helps low-income people with disabilities pay for home modifications to rental properties. Landlords can receive grants to make accessibility improvements, which benefits tenants. This works differently than homeowner programs because it focuses on rental housing.

State vocational rehabilitation agencies sometimes fund home modifications as part of employment services. If a modification would enable someone to work, it may be covered. For example, a person recovering from a spinal cord injury might receive funding for a ramp and accessible bathroom modifications if these changes allow return to work.

The Assistive Technology Act, administered by states, creates programs that help people learn about, try, and sometimes borrow assistive technology and equipment. Many state AT programs maintain equipment loan libraries where people can try devices before purchasing. Some programs also provide refurbished equipment at reduced cost.

Veterans may access home modification funding through the VA's Specially Adapted Housing program for service-connected disabilities. This program can cover substantial modifications, including ramps, widened doorways, accessible kitchens and bathrooms, and elevator installation in some cases.

Nonprofit organizations, disability organizations, and community foundations sometimes fund specific modifications. Organizations focused on particular disabilities—spinal cord injury organizations, muscular dystrophy associations, cerebral palsy organizations, and similar groups—sometimes maintain funds for home accessibility improvements for their constituents.

Practical takeaway: Document what modifications would improve your mobility and safety at home, then research whether your situation might qualify for assistance through vocational rehabilitation, housing programs, or nonprofit organizations focused on your particular disability or condition.
