Understanding Mouth Ulcers: What They Are and Why They Happen

Mouth ulcers, also called canker sores or aphthous ulcers, are small painful sores that develop inside your mouth. These sores typically appear on the soft tissues inside your cheeks, lips, tongue, or at the base of your gums. Unlike cold sores, which are caused by the herpes simplex virus and appear on the outside of the mouth, canker sores are not contagious and develop for different reasons.

The exact cause of mouth ulcers isn't always clear, but research shows several common triggers. Minor injuries from dental work, aggressive brushing, accidental cheek biting, or sharp foods like chips or hard candy can damage the mouth's delicate lining and lead to ulcer formation. Some people develop ulcers after eating acidic or spicy foods such as citrus fruits, pineapple, tomatoes, or hot peppers. Nutritional deficiencies also play a role—ulcers appear more frequently in people lacking vitamin B12, zinc, folate, or iron.

Stress and hormonal changes are significant contributors. Studies show that approximately 60% of people who experience recurrent mouth ulcers report stress as a trigger. Women often notice ulcers developing at specific points in their menstrual cycle. Certain autoimmune conditions, such as celiac disease, Crohn's disease, or Behçet's disease, cause frequent ulcers. Additionally, sodium lauryl sulfate (SLS), a foaming ingredient in many toothpastes, can trigger ulcer development in sensitive individuals.

Most mouth ulcers are minor and heal within one to two weeks without leaving scars. However, larger ulcers called major aphthous ulcers can be deeper and take three to six weeks to heal. Herpetiform ulcers appear as clusters of tiny sores and may merge into larger ones. Understanding what type of ulcer you have and what triggered it is the first step toward prevention and treatment.

Practical Takeaway: Keep a simple log noting when ulcers appear, what you ate that day, your stress level, and where the sore developed. Over time, patterns emerge that help you identify your personal triggers, whether they're specific foods, toothpaste ingredients, or stress periods.

Over-the-Counter Treatment Options and How They Work

Several types of over-the-counter products can reduce ulcer pain and potentially speed healing. Topical treatments applied directly to the sore work fastest because they reach the affected area immediately. Benzocaine-containing products like Orajel provide temporary numbing that typically lasts 15 to 20 minutes, making eating and speaking more manageable. These products work by blocking nerve signals in the affected area, not by treating the underlying ulcer.

Antimicrobial mouth rinses containing chlorhexidine or hydrogen peroxide reduce bacteria in your mouth and may lower infection risk. Research indicates that rinsing with antimicrobial solutions two to three times daily can modestly reduce ulcer healing time. Hydrogen peroxide rinses work by breaking down dead tissue and killing bacteria, creating an environment more favorable for healing. Use a 3% solution diluted according to product directions, typically for 30 to 60 seconds per rinse.

Topical corticosteroid products like triamcinolone can reduce inflammation and pain when applied directly to the ulcer. These work best when applied multiple times daily before the ulcer fully forms. Some studies show topical steroids reduce healing time from 7-10 days to 5-7 days when used early. However, they work best on ulcers in early stages and are less effective on fully developed sores.

Protective barrier products create a seal over the ulcer, reducing irritation from food and saliva. Pastes and gels containing carboxymethylcellulose or similar ingredients stick to the sore and provide a physical barrier. These products are particularly helpful for ulcers on the lower lip or inside the cheek where they contact food during eating. They typically remain in place for 2 to 8 hours depending on the product and your saliva production.

Oral medications like sucralfate suspension coat the entire mouth and may provide longer-lasting protection than topical products. Amlexanox paste, available by prescription, reduces healing time more effectively than over-the-counter options—studies show it can reduce healing time by 1 to 2 days when applied four times daily for up to 10 days.

Practical Takeaway: Different products work better at different ulcer stages. Use barrier products once the ulcer is fully formed to protect it during eating. Apply topical medications in early stages when pain first develops. Try antimicrobial rinses multiple times daily throughout the ulcer's life to reduce infection risk and support faster healing.

Natural Remedies and Home Treatments That May Help

Many people find relief using natural substances available in most homes. Salt water rinses are among the oldest and most widely used remedies, and research supports their effectiveness. Mix one-half teaspoon of salt in 8 ounces of warm water and rinse for 30 to 60 seconds several times daily. Salt water works by reducing bacteria, pulling fluid out of inflamed tissue to reduce swelling, and creating an environment less favorable for secondary infections. Repeat this rinse after meals and before bedtime for best results.

Honey has antimicrobial and anti-inflammatory properties supported by scientific research. Applying raw or medicinal honey directly to the ulcer several times daily may reduce pain and support healing. A study published in the journal Phytotherapy Research found that honey reduced ulcer healing time compared to conventional treatments. Apply a small amount directly to the sore using a clean finger or cotton swab, allowing it to sit for several minutes before eating or drinking.

Milk of magnesia, commonly used for stomach issues, can soothe ulcers when applied topically. This alkaline liquid neutralizes acids in your mouth that irritate the sore. Dab a small amount directly on the ulcer several times daily, particularly before meals. Unlike topical anesthetics that only mask pain, milk of magnesia may reduce the underlying irritation causing the pain.

Baking soda has mild antiseptic properties and can reduce inflammation. Mix one-half teaspoon of baking soda with a small amount of water to create a paste, then apply it directly to the ulcer. Alternatively, rinse with a solution of one-half teaspoon baking soda in 8 ounces of water. Repeat several times daily. Baking soda's alkaline nature helps neutralize mouth acids and create a less irritating environment.

Ice provides temporary numbing relief similar to benzocaine products but without chemicals. Hold an ice cube against the ulcer for 5 to 10 minutes. Repeat several times daily, especially before meals or when pain is most noticeable. This works best for recently formed ulcers when inflammation is highest. The cold reduces blood flow to the area, decreasing inflammation and pain signals.

Chamomile tea contains compounds with anti-inflammatory properties. Brew a tea bag in hot water, let it cool slightly, then place the wet tea bag directly against the ulcer for 5 to 10 minutes. Alternatively, rinse your mouth with cooled chamomile tea several times daily. Some people find that the tannins in tea have astringent properties that help reduce swelling and pain.

Practical Takeaway: Natural remedies work best when used consistently multiple times daily. Combine approaches—for example, use salt water rinses in the morning and evening, apply honey after meals, and use ice when pain is worst. Most natural remedies reduce symptoms rather than dramatically speed healing, but they're inexpensive, safe, and often effective enough to manage discomfort without medication.

Dietary Adjustments and Foods to Avoid During Healing

What you eat significantly affects ulcer pain and healing speed. Certain foods trigger additional irritation or create an environment that slows healing. Acidic foods and beverages—including citrus fruits, tomatoes, pineapple, strawberries, vinegar, and soda—lower your mouth's pH, creating an acidic environment that irritates open sores. If you enjoy acidic foods, consume them with meals rather than alone, which reduces the acid concentration. Rinse your mouth with water or milk afterward to neutralize
