Understanding Diarrhea: Causes and When It Becomes a Concern

Diarrhea is one of the most common digestive problems people experience. According to the CDC, the average American adult experiences diarrhea about once per year, and children experience it even more frequently. Diarrhea occurs when stool passes through the intestines too quickly, preventing your body from absorbing enough water. This results in loose, watery bowel movements that may occur multiple times throughout the day.

Several factors can trigger diarrhea. Viral infections like norovirus and rotavirus are among the most common culprits, particularly during winter months. Bacterial infections from contaminated food or water—such as Salmonella or E. coli—cause diarrhea in roughly 48 million Americans annually. Parasitic infections, food intolerances (especially lactose intolerance), medications like antibiotics, and sudden diet changes can all lead to loose stools. Inflammatory bowel conditions like Crohn's disease and ulcerative colitis produce chronic diarrhea lasting weeks or months.

Most cases of acute diarrhea resolve within 24 to 48 hours without specific treatment. However, certain situations warrant medical attention. Seek care if diarrhea lasts more than two days in adults or 24 hours in children, if you experience severe abdominal pain, bloody stools, signs of dehydration (extreme thirst, dark urine, dizziness), or if you have a weakened immune system. For children under six months, elderly adults, and people with chronic illnesses, even mild diarrhea should be evaluated by a healthcare provider.

Practical Takeaway: Understanding whether your diarrhea is likely viral, bacterial, or related to diet helps you determine the appropriate response. Most cases resolve on their own, but certain red flags require professional evaluation.

Rehydration: The Most Important First Step

Rehydration is the single most critical action when managing diarrhea. When you lose fluids through watery stools, your body becomes depleted of both water and essential electrolytes—minerals like sodium, potassium, and chloride that regulate nerve function, muscle contractions, and fluid balance. The World Health Organization identifies dehydration as the primary danger during diarrheal illness, particularly in children and older adults.

Plain water alone is insufficient for effective rehydration during diarrhea. While water replaces lost fluids, it doesn't replace electrolytes, which can actually lead to a dangerous condition called hyponatremia if consumed in excess without electrolyte replacement. Oral rehydration solutions (ORS) combine water with precise ratios of glucose, sodium, and potassium to restore what your body lost. These solutions are specifically formulated to be absorbed efficiently by your intestines even when diarrhea is ongoing. Common ORS products include Pedialyte, WHO Oral Rehydration Salts, and store brands like Walmart's Equate brand, which cost between $3 to $8 per package.

If ORS products aren't available, you can make a simple rehydration solution at home using ingredients you likely have: mix six teaspoons of sugar and half a teaspoon of salt in one liter of clean water. This approximates the glucose-to-electrolyte ratio found in commercial solutions. You can also use coconut water, which naturally contains electrolytes and is tolerated well by most people during digestive upset. Sports drinks like Gatorade contain electrolytes but have higher sugar content than ideal; if using them, dilute them with water at a 1:1 ratio.

Drink small, frequent amounts rather than large quantities at once. Sipping 5 to 10 milliliters (about one teaspoon) every few minutes works better than drinking 250 milliliters (8 ounces) all at once, which can trigger more diarrhea. A good guideline is to consume 75 to 100 milliliters per kilogram of body weight daily during acute diarrhea. For a 70-kilogram (154-pound) adult, this means approximately 5 to 7 liters per day spread throughout waking hours.

Practical Takeaway: Keep oral rehydration solutions on hand at home. If you develop diarrhea, begin sipping small amounts immediately—this single action prevents most diarrhea-related complications.

Dietary Management: What to Eat and What to Avoid

Your diet significantly influences how quickly diarrhea resolves. During the acute phase, the most common medical recommendation is the BRAT diet—bananas, rice, applesauce, and toast—which consists of bland, low-fiber foods that are easy to digest and less likely to stimulate the intestines. These foods also contain pectin and resistant starch, compounds that add bulk to stool and slow intestinal movement. Modern gastroenterology also recommends including lean proteins and other bland foods as tolerated, recognizing that a slightly more varied diet doesn't slow recovery.

Foods to prioritize during diarrhea include plain crackers, dry cereal, plain chicken or turkey, white fish, eggs, plain pasta, white bread, and cooked vegetables without added fat. These foods are low in fat and fiber, both of which can worsen diarrhea by stimulating intestinal contractions. Bananas deserve special mention—one medium banana provides 422 milligrams of potassium, helping replace this electrolyte lost during diarrhea. Rice is equally valuable; white rice is easier to digest than brown rice because it lacks the outer bran layer that stimulates bowel movements.

Foods to strictly avoid include dairy products (milk, cheese, ice cream), high-fat foods (fried foods, fatty meats, butter), high-fiber foods (whole grains, raw vegetables, beans, nuts), sugary foods (candy, cookies, sodas), caffeine (coffee, tea, cola), and spicy foods. These categories all trigger or worsen diarrhea by stimulating intestinal movement or exceeding your intestines' current absorption capacity. Lactose intolerance often develops temporarily during diarrheal illness because the intestinal lining becomes damaged and produces less lactase enzyme. This typically resolves within a few days to weeks after diarrhea stops.

Timing matters significantly. Eat small, frequent meals every two to three hours rather than three large meals. This approach provides steady nutrition without overwhelming your digestive system. Many people find that starting with clear broths, crackers, and rice for the first 12 to 24 hours, then gradually introducing bland proteins and toast, works well. As symptoms improve over several days, you can slowly expand your diet back to normal foods, reintroducing fiber and dairy last.

Practical Takeaway: Stock your pantry with BRAT diet staples and bland proteins. When diarrhea strikes, begin with the most restricted diet and gradually expand as you improve rather than immediately returning to your normal diet.

Over-the-Counter Medications and Supplements

Several over-the-counter medications can reduce diarrhea symptoms, though understanding which ones work and when to use them is important. Antimotility agents like loperamide (Imodium) and diphenoxylate (Lomotil) slow intestinal contractions, reducing stool frequency. These work effectively for non-infectious diarrhea—they can reduce stool output by 80 to 90 percent within one to two hours. However, antimotility medications should never be used for bacterial or parasitic infections, as they can trap pathogens in your intestines and worsen the infection. They're also inappropriate for diarrhea with bloody stools or high fever, warning signs of bacterial infection.

Bismuth subsalicylate (Pepto-Bismol, Kaopectate) works differently—it reduces intestinal inflammation and has mild antimicrobial properties. It's generally safer than antimotility agents for infectious diarrhea, though it can cause black stools (harmless) and shouldn't be used by people allergic to aspirin or those taking blood thinners. Typical dosing is one tablet or 30 milliliters every 30 minutes to one hour, up to eight doses daily, for a maximum of two days without medical guidance.
