Understanding TB Testing and Why It Matters

Tuberculosis (TB) is a serious infection caused by bacteria that spread through the air when an infected person coughs, sneezes, or speaks. According to the Centers for Disease Control and Prevention (CDC), about 2.5 billion people worldwide have latent TB infection, meaning the bacteria is in their body but not making them sick. In the United States, approximately 7,700 TB cases were reported in 2022, though this number has been declining over the past two decades.

TB testing identifies whether someone has active TB disease or latent TB infection. Active TB disease means the bacteria are multiplying and causing illness, with symptoms like persistent cough, fever, night sweats, and weight loss. Latent TB infection means the bacteria are present but dormant, and the person cannot spread it to others. However, latent TB can progress to active disease, particularly in people with weakened immune systems.

Testing is important because it allows doctors to diagnose TB early and start treatment before the disease becomes severe or spreads to others. People who have been in close contact with someone who has active TB, healthcare workers, people living with HIV, and those from countries with high TB rates are considered at higher risk and may benefit from testing.

There are two main types of TB tests: skin tests (also called tuberculin skin tests or TST) and blood tests (also called interferon-gamma release assays or IGRAs). Both tests require a healthcare provider to interpret the results. Skin tests involve injecting a small amount of protein under the skin and checking the reaction 48 to 72 hours later. Blood tests measure how the immune system responds to TB antigens and can provide results within one to two days.

Practical Takeaway: Understanding what TB testing detects and why testing matters helps you recognize situations where testing might be recommended. If you have symptoms of TB or have been exposed to someone with active TB, seeking testing through a healthcare provider is an important step toward diagnosis and treatment.

Where to Find TB Testing Through Public Health Departments

State and local health departments offer TB testing services to the public, often at little or no cost. These departments maintain TB control programs and have trained staff who can perform tests and explain results. Most health departments operate TB clinics during business hours, and many offer testing at multiple locations throughout their service areas.

To locate your state health department, visit the Association of State and Territorial Health Officials website or search your state's name plus "department of health." Once you find your state agency, look for the TB program or communicable disease section. State health departments maintain lists of clinics and testing sites, which are usually available on their websites or by phone. You can also call 2-1-1 (a national helpline) to receive information about TB testing locations in your area, as this service connects people to local health resources.

County and local health departments often provide more convenient testing locations than state offices. Many counties operate multiple clinics in different neighborhoods or community health centers. For example, in Los Angeles County, the Department of Public Health operates TB clinics at various locations throughout the county, and services are provided in multiple languages. Chicago's Department of Public Health offers TB testing at six primary clinics plus several satellite locations.

Public health department TB clinics typically do not require advance notice for walk-in testing, though some locations ask you to call ahead to confirm hours or reduce wait times. Staff can answer questions about test results, provide information about TB disease, and refer you to additional medical services if needed. Many health departments also offer TB testing through their disease investigation services, meaning if you report TB symptoms or exposure to a case investigator, they will arrange testing for you.

When visiting a public health TB clinic, bring a form of identification and any insurance information you have, though lack of insurance will not prevent you from receiving testing. If you are experiencing TB symptoms or were recently exposed to someone with active TB, inform staff when you arrive so they can prioritize your testing.

Practical Takeaway: Public health departments are a reliable first place to start looking for TB testing. Contact your local or state health department to learn about clinic locations, hours, and what to bring when you visit for testing.

Finding TB Testing at Community Health Centers and Primary Care Providers

Federally Qualified Health Centers (FQHCs), also called community health centers, provide TB testing alongside other primary care services at locations throughout the country. These centers serve patients regardless of ability to pay and often offer services on a sliding fee scale based on income. The Health Resources and Services Administration (HRSA) maintains a searchable database of FQHCs at findahealthcenter.hrsa.gov, where you can enter your address and see centers near you.

Community health centers typically have one or more healthcare providers on staff who can order TB tests. If you visit a center for other health concerns, you can ask whether TB testing is available. Many centers can perform skin tests on-site and have arrangements with laboratories for blood tests. Testing at community health centers may require a brief visit with a nurse or provider to discuss your health history and reasons for testing, which helps ensure the right test is ordered for your situation.

Primary care doctors, including family medicine physicians, internists, and pediatricians, can order and sometimes perform TB testing in their offices. If you have a regular doctor, calling to ask about TB testing is a straightforward way to arrange testing. Some offices perform skin tests during your visit, while others order blood tests that you can have done at a lab. Insurance often covers TB testing when ordered by your doctor, though you should confirm your coverage by calling your insurance provider or checking your policy documents.

Urgent care centers and retail clinics (such as those located in pharmacies) sometimes offer TB testing, though availability varies by location. These settings are convenient for quick testing if you cannot visit a health department or primary care office. However, retail clinics may not have providers available to interpret skin test results on-site, which means you may need to return to the clinic 48 to 72 hours after a skin test is placed to have results read by a healthcare provider.

When contacting a doctor's office or clinic to ask about TB testing, you can ask directly whether they offer testing and what the process involves. Questions to ask include whether they can perform the test you need (skin test or blood test), when results will be available, how much the test costs, and whether your insurance is accepted.

Practical Takeaway: Your primary care provider or a nearby community health center can often order TB testing as part of their services. Use the HRSA health center finder to locate community health centers in your area, or call your regular doctor to learn whether they offer TB testing.

TB Testing at Hospitals, Urgent Care, and Emergency Departments

Hospitals provide TB testing through emergency departments, urgent care units, and inpatient services. If you visit a hospital for any reason, staff can arrange TB testing if it is medically necessary. For example, if you arrive at an emergency department with symptoms that could indicate TB (such as prolonged cough, fever, or chest pain), the provider will likely order a TB test as part of your evaluation.

Hospital-based TB testing is useful when you need rapid evaluation and you cannot wait for an appointment elsewhere. Emergency departments have access to laboratory services and providers who can interpret results quickly. However, using an emergency department for routine TB testing when you have no symptoms is not necessary and results in higher costs. Emergency department care is most appropriate when you have acute symptoms or when testing is needed as part of hospitalization.

Urgent care centers can perform or order TB tests for patients who come in with symptoms or those seeking preventive testing. These centers typically have extended hours compared to doctor's offices, making them accessible outside standard business hours. Similar to retail clinics, urgent care centers may order blood tests that require follow-up lab visits, or they may perform skin tests and ask you to return for result reading.

If you are admitted to a hospital for any reason, hospital staff will likely perform or order a TB test as part of your admission screening. This is standard procedure in most hospitals to identify patients who may have active TB and to protect other patients and staff. You can ask your hospital care team whether TB testing will be part of your hospitalization and what results mean for your treatment.

When visiting urgent care or an emergency department, inform staff of any TB symptoms you have, recent exposure to TB, or reasons you believe you need TB testing. This information helps providers determine whether testing is necessary and which type of test is most appropriate. If you are sent to an emergency department and do not have an active health condition requiring emergency care, the cost of testing may
